ABSTRACT

Gastric cancer is one of the most common cancers in the world that can metastasize to other sites. The most common sites of metastasis are liver, lung, bone, and adrenal glands. Skin metastasis in gastric cancer is rare, and it usually occurs after diagnosis of primary cancer [1]. The patient reported here is a 65-years old male with gastric adenocarcinoma that in the course of chemotherapy came down with some skin lesions. The biopsy report points to skin metastasis from gastric cancer. In spite of good systemic response to chemotherapy, the lesions progressed. Skin metastasis has poor prognosis, and it is a sign of disease diffusion [2]

ABSTRACT

This article outlines the Government Facilitator Roles in Ecopreneurship functions in Environmental NGOs. Target Group was environmental NGOs in Iran [Key actors, committed to preserving ecological and environmental integrity]. Accelerating innovation way for improving the environmental conservation by Environmental NGOs is the conceptual mean of Ecopreneurship in this research. Questionnaire as a research instrument was to measure the constructs. Content validity by panel study confirmed. To determine reliability alpha coefficient was calculated. Value ranged from 0.74 to .96 showed that research instrument has acceptable capability to collect data and satisfy accepted condition. Dominant statistical method was SEM and for data analysis PLS-Graph was employed. According to path analysis results, four dimensions of Government facilitator have a significant role in Ecopreneurship in Iranian environmental NGOs. End of article some of implications were illustrated for improving Ecopreneurship functions by Government facilitator roles

ABSTRACT

The current research was carried out with the aim of the investigation of organizational learning dimensions role in the development of enviropreneurship in environmental Non-governmental organizations [NGOs] in Iran by descriptive correlation method. Statistical population of this research was including all environmental NGOs of industries in all over the country and statistical samples were extracted by random sampling and sampling formula. Finally, 51 environmental NGOs were selected among active environmental NGOs as the studied sample. The main instrument of data collection in this research was questionnaire used for measuring enviropreneurship and organizational learning. To determine the validity of questionnaire, face validity method and opinion poll from experts were used. The designed instrument was at first used in the population out of pre-test statistical sample and Cronbach's alpha was applied to determine reliability. Cronbach's alpha for enviropreneurship was 0.76 and 0.97 for organizational learning. The main technique of data processing was logistic regression and SPSS software version 18 was used in this regard. Generally, the results of logit function showed that among organizational learning dimensions, organization connection with environment, encouraging co-workers and team learning and continuous learning opportunities were the most important variables determining the condition of enviropreneurship in environmental NGOs

ABSTRACT

Gastric cancer is the second common cancer in Iran and it has unfortunately no diagnostic signs in early stages. Considering the role of genetic and environmental factors in the development of gastric cancer, this study aimed at evaluating DNA-content of stomach cells in patients with gastric cancer in order to find a reliable index for prediction of disease prognosis and providing more exact therapeutic procedure. A total of 22 samples taken from patients with gastric cancer were evaluated. Peritoneal lavage cells were centrifuged and stained with DNA-staining kit. Then DNA-content was analyzed and compared with that in normal cells by employing flow cytometry and using modfit software. In whole, 12 out of 22 samples were positive and DNA aneuploidy was present in 33.3%. SPF increase was observed in 22.2% of positive cases. SPF can be used as a reliable indicator in pathological staging of cancer and consequently in decision making for treatment

ABSTRACT

Gastric surgeries are still among the major causes of surgery complications. Study on prevalence of these complications and recognition of their risk factors are strongly needed. To study Gastric surgery complications and strategies for their management in patients hospitalized at Ayatollah Taleghani hospital. In this Cross-sectional study all gastric surgeries at Ayatollah Taleghani hospital between 1994 and 2002 were assessed. Variables were age, sex, type of surgery, indication of surgery, type of complication and management strategy [medical or surgical]. Data were collected from patients' medical records. Indications for surgery were gastric malignancy and neoplasia in 277 patients [59.8%], peptic ulcer in 151 [32.6%], and other causes in 7.6%. The most prevalent surgeries were Billroth II [23%] and total gastrectomy [15%]. The highest level of complications was observed in Billroth II [24%]. Complications of surgery occurred in 42 cases [9%] among those 33 males and 9 females. Except for vagotomy and pyloroplasty, less than 50% of complications needed surgical management. This study showed that the prevalence of gastric surgeries complications is 9%, comparable to those found in other studies. The most prevalent surgeries were Billroth II and total gastrectomy, believed to be associated with relatively higher level of complications

ABSTRACT

Kidney transplantation from a live donor has many advantages, but the donor should undergo a major surgery and accept potential morbidity and mortality. In this study, we evaluated the long term effects of nephrecetomy on renal function and blood pressure of kidney donors. In this cross-sectinal study, we assessed 195 kidney donors. The controls were 100 healthy individuals with two kidneys. The variables were age, sex, systolic blood pressure [SBP], diastolic blood pressure [DBP], serum creatinine, creatinine clearance and proteinuria [all before and after kidney donation] and time interval between donation and study. Data analysis was accomplished using SPSS software, and paired t-test and Pearson correlation coefficient were used for data analysis. Although serum creatinine was elevated at the time of study, but this rise was not significant. SBP at the time of study was significantly greater than the time of donation [P<0.001]. Creatinine clearance at the time of study was significantly lower than the time of donation [P<0.001]. There was no correlation between serum creatinine, creatinine clearance, SBP, DBP and age at the time of kidney donation in men. Age at the time of kidney donation was correlated with SBP and DBP and creatinine changes in women [P=0.001 and P=0.002, respectively]. This study showed that nephrectomy does not impair renal function in donor and does not lead to a progressive rise in blood pressure or urine protein excretion up to 6.5 years after nephrectomy. Nevertheless, we suggest that kidney donors should be selected after exact screening for potential risk factors of renal disease

ABSTRACT

Tail gut cyst [TGC] is a non common cystic lesion with post anal origin which is occurred in retro rectal region. In this article we studied a 90-year-old man with a tail gut cyst who presented with post sacral ulcer. The patient did not cooperate well with taking history. For further evaluations the patient underwent surgical treatment for detriment and the region was drained off and repaired. At operation retro rectal cystic mass was excised for pathologic studies. Although the incidence of malignancy in TGC is extremely rare, it should be considered in differential diagnosis of retro rectal tumors especially in middle-aged women

ABSTRACT

A 35-year woman, from Zahedan, was admitted to Shaheed Sadoughi hospital for abdominal pain. She was suffering from a sudden left flank pain during 3 weeks ago and bloody stool from two days before reffering to the hospital. After performing paraclinical evaluations, including routine laboratory exams, plain abdominal radiography and CT scan, she underwent left radical nephrectomy with the diagnosis of a retropritoneal mass. Pathologic studies found renal angiomyolipoma with involvement of omentum. As we didn't find a similar case in the search of medical databases, it seems that this is the first report of renal angiomyolipoma with involvement of omentum to our knowledge

ABSTRACT

This study was performed to report the ultrasonographic finding and final diagnosis of a group of primary amenorrhea patients. Pelvic ultrasonography [US] was employed as the first diagnostic modality to evaluate primary amenorrhea in 53 patients who were admitted to gynecology or endocrinology clinics at Taleghani hospital from 2002 to 2006. US was based upon the presence or absence of the uterus and ovaries and any other abnormal sonographic findings. Karyotype analysis was also performed for all the patients. The uterus was not visualized in 16 [30%] patients: due to mllerian agenesis in 14 and testicular feminization and true hermaphroditism in two other patients. Mllerian anomalies with hematometrocolpos or hematometra were seen in 5 [9%] patients. Thirty-two [60%] patients had a normal or hypoplastic uterus. Pelvic US showed that ovaries were in normal limits in 39 [73%] patients; they were not visible in 9 patients. The report of pelvic US was not conclusive in 3 patients; 2 had an ovarian tumor or cyst. Irrespective of the presence or absence of the uterus, all patients with visible ovaries [except one] had a normal karyotype. US of the pelvis can be the initial diagnostic modality. Based on US findings, we can make decision for further work ups; there is no need to perform all paraclinical investigations for each patient

ABSTRACT

Sjogren's syndrome [SS] is a systemic autoimmune disease that presents with sicca symptoms of the main mucosal surfaces. The spectrum of disease extends from sicca syndrome to systemic involvement and may be complicated by the development of lymphoma. Many types of malignant and pseudo malignant diseases have been reported, but the most important is non Hodgkin lymphoma. We here present a 45 year-old woman with SS that referred with pancytopenia. By molecular and histochemical techniques a diagnosis of acute myeloblastic leukemia of M3 type [AML-M3] was made for this patient

ABSTRACT

Primary hyperparathyroidism is a quite frequent disorder however, giant parathyroid adenoma is a rare entity. We described a 39-year old woman presenting with lower extremities bone pain within the recent 3 years. She had been receiving levothyroxin during the past 10 years due to hypothyroidism, meanwhile, she had a bulge right thyroid lobe. Laboratory examinations revealed she was euthyroid, however, scanning showed parathyroid adenoma. During surgery, an adenoma measured 2x3x6cm, weighted 22gr was resected. Giant parathyroid adenoma present with non-characteristic symptoms, thus, routine laboratory measurements including serum calcium and phosphorous should be proposed for suspected individuals

ABSTRACT

Hypertension is one of the most prevalent diseases that, causes lots of complications, it is not controlled. Sufferers should change their life style and avoid the risk factors. The aim of this research is to determine the hypertension rate and it's risk factors in elders of Yasouj in 2005. This is a descriptive analytical and cross-sectional study. Convenient sampling was done which was consisting of all elders above 60 referred to the health centers during the first six months of the year. Data collection instrument was questionnaire and also scale, meter and sphygmomanometer was used to determine weight, height and blood pressure. Collected data were analyzed using SPSS software and descriptive and inferential statistics such as ; analysis of variance and x2 tests. 286[47 .4%] of elders were suffering from HTN out of which 233[38.6%] had HTN history and 53[8.8%], did not have any knowledge of their HTN. The BMI mean in elders with high blood pressure was 94.1 +/- 3.3 and in others, it was 23.1 +/- 2.3. The average weekly use of meat, eggs and smoking of patients with high blood pressure as compare to those with normal blood pressure did not show significant difference. Considering the risk factors of hypertension the necessity of screening and educating elders by health personnels to improve their life style is felt
